Re premiums and spreads, if the bid/offer spread is wider than about 1%, then you need to set your own price - don't buy on 'at best'. This is particularly true for Lindsell Train, but probably the others except PNL too.
As for premiums, they have all been at premiums for a considerable time, I don't think it is a major concern.
